Steps to Apply

  2. If you are completing training within the next six months, you may use a form letter instead of a traditional sponsorship letter (See Sponsorship Letter Requirements below). If you completed your training in 2015 or 2016, you must submit two letters of sponsorship.

Processing Your Application

Please note that you can apply up to six months prior to the completion of your training. However, if you apply before you pass your Board exams and before you have a confirmed hospital and/or academic appointment, your application will remain on file with a status of “pending” until we receive confirmation that you have passed the Boards and have confirmed appointments. Additionally, if you are returning for further subspecialty training, you are not eligible to apply for FACC until you are closer to completing that training.

Applications are accepted on a rolling basis, but are processed during two key deadline periods:

  • Applications received before May 1 will be reviewed in July
  • Applications received before Oct 1 will be reviewed in Oct
  • Note: Most applicants who will complete training later this year and apply now will be reviewed with the October deadline, after all confirmations are received.

Your Sponsors and Sponsorship Letters

Applicants who completed their fellowship training in 2015 or 2016 are required to submit two letters of sponsorship from current FACCs.

Sponsorship Letter Requirements:

  • Letters must be included with your application
  • If you are completing training within the next six months, you may use this form letter >>>

Should you choose not to use the form letter, your letter must adhere to the following:

  • Letters must be on the sponsor’s business letterhead, dated and signed by the sponsor
  • Letters should detail your accomplishments of distinction, such as hospital, medical society or educational leadership
  • Letters should be original in content—substantially identical letters will be rejected

Sponsor Requirements:

  • Sponsors must be current FACCs
  • Sponsors must be well acquainted with your current professional activities
  • At least one sponsor must be from your geographic are of professional activity
  • Only one sponsor can be from a partner or colleague that works in your office
  • Only one sponsor can be from your training program
  • Your relatives may not sponsor you

*Offer valid for those who completed their cardiovascular fellowship training from 6/30/2015 through present OR who will complete their cardiovascular training within the next six months. Offer valid on $150 application fee waiver only. The application will be processed as "pending" until training completion, post-training academic and/or hospital appointment begins and attainment of the cardiovascular specialist Board certification. Once all information is received, applications will be processed.
