1.  Download the application
2.  Fill out the sponsor letter
3.  Complete the application and sponsor letter and mail/fax to the ACC Member Relations Department with the following documents:

1. $125 check or credit card number with expiration date ($100 annual dues for 2006, including the remaining months of 2005 at no additional charge plus $25 non-refundable processing fee).
2. A copy of the applicant's RN degree for applicants who are RN, CNS or NP or a copy of PA certificate from a program accredited by ARC-PA or a predecessor agency or NCCPA certified for PA applicants.
3. A copy of the license to practice in the state. Federally employed PAs should provide NCCPA certification in lieu of a license.
4. The letter of sponsorship signed by a Fellow of the American College of Cardiology (FACC).  Click here to print the Sponsor Letter.
