Download an application, complete and mail to ACC Member Relations Department with the following documents:

1. $125 check or credit card number with expiration date (for new member and $100 for renewing members).
2. A copy of the applicant's RN degree for applicants who are RN, CNS or NP, a copy of PA certificate from a program accredited by ARC-PA or a predecessor agency or NCCPA certified for PA applicants, or a copy of PharmD degree.
3. A copy of the license to practice in the state. Federally employed PAs should provide NCCPA certification in lieu of a license.
4. A completed/signed letter of sponsorship from a Fellow of the American College of Cardiology (FACC). (Sponsor Letter)
