Disease Management
Disease management (DM) is a process of organizing care for specific high-cost and/or high volume conditions, with the intention of improving outcomes and, when possible, lowering overall costs. Its presence has exploded onto the payer realm for quality and performance measures across all forms of health care payment. For questions related to DM activities please contact Payer Advocacy, advocacydiv@acc.org.
