What is H2H?

The Hospital to Home (H2H) Quality Improvement Initiative is a national effort to reduce preventable readmissions for patients recently hospitalized with a cardiovascular condition—the leading cause of hospitalization in the United States. Its goal: to focus on "excellence in transitions" by reducing the 30-day, all-cause hospital readmission rates among patients discharged with heart failure or acute myocardial infarction (AMI) by 20 percent nationally by December 2012.

The H2H initiative is a joint effort between the American College of Cardiology and the Institute for Healthcare Improvement to improve the transition of patients from inpatient to outpatient status. It is centered on three guiding principles, which are in alignment with the National Quality Forum Care Transitions Measures:

    1. Is the patient familiar and competent with their medications and do they have access to them?
    2. Does the patient have a follow-up visit scheduled within a week of discharge and are they able to get there?
    3. Does the patient fully comprehend signs and symptoms that require medical attention and know who to contact if they occur?
