    I12 - Hypertensive chronic kidney disease

    • I12.0 - Hypertensive chronic kidney disease with stage 5 chronic kidney disease or end stage renal disease
    • I12.9 - Hypertensive chronic kidney disease with stage 1 through stage 4 chronic kidney disease, or unspecified chronic kidney disease

    I13 - Hypertensive heart and chronic kidney disease

    Hypertension with Heart Disease (see guideline I.C.9.a.1 for complete information)

    I11 Hypertensive heart disease, when a causal relationship is stated (due to hypertension) or implied (Hypertensive) use an additional code from category I50 – Heart failure, to identify the type of heart failure in those patient with heart failure. If the same two conditions are mentioned but it is not stated that a causal relationship is present then the two conditions are coded separately.

    Hypertensive Chronic Kidney Disease (I.C.9.a.2 for complete information)

    In the I12 codes it is stated that these codes can be assigned when both hypertension and a condition classifiable to category N18, Chronic kidney disease (CKD), are present. Unlike hypertension with heart disease, ICD-10-CM presumes a cause-and-effect relationship between CKD and hypertension. An additional code is assigned to describe the stage of the chronic kidney disease.

    Hypertensive Heart and Chronic Kidney Disease (I.C.9.a.3 for complete information)

    Assign codes from combination category I13, when both hypertensive kidney disease and hypertensive heart disease are stated in the diagnosis. Here you can also assume a relationship between the hypertension and the chronic kidney disease, whether or not the condition is so designated. If heart failure is present, assign an additional code from category I50 to identify the type of heart failure.

    Hypertension, Secondary

    Secondary hypertension is due to an underlying condition so two codes will be required.
