First Patient Navigator Hospitals Unveiled
The Pulse of ACC | The ACC has released the names of the first hospitals participating in the first phase of its new Patient Navigator Program.
Participating hospitals will receive funding to establish a team of caregivers focused on helping patients overcome challenges during their hospital stay and in the weeks following discharge when they are at most risk for readmission. AstraZeneca is the founding sponsor of the program.
“These hospitals are serving as pioneers in a new approach to heart disease treatment and care that puts emphasis on meeting patients’ ongoing needs and helping patients make a seamless transition from the hospital to the home,” said ACC President John Gordon Harold, MD, MACC.
