eDRCF Submission Details
The CMS Quality Report from CathPCI Registry data for 2017 supports Individual reporting. The physician who’s NPI number appears on the report must sign this electronic Data Release Consent Form (eDRCF). In so doing you attest that you have reviewed the Report and verified that at least 50 percent of your eligible patients meet the denominator criteria of the QCDR Report measures, regardless of payer.
Please complete and/or verify all fields. Specifically, at least one practice Tax Identification Number (TIN) and the address (Street, City, State and Zip code) must be entered.
Once the eDRCF has been signed there is no opportunity to opt out or halt the report submission to CMS on your behalf. Note, the outcome of the report will reflect the decision by CMS and the physician will work with CMS through the informal review process to address any questions or concerns regarding the report and its impact on the Merit-Based Incentive Payment System (MIPS) score.
You have signed your eDRCF, it will be counter-signed by ACC within the next day. Please check back tomorrow when you will be able to download and save the fully-executed eDRCF from the physician dashboard landing page . If you receive this message over several days please contact our NCDR Business Support Team at 800-257-4737 or mail to firstname.lastname@example.org.

Sorry we seem to have some trouble generating your eDRCF. Please try again, if this problem persists please try again tomorrow.
