ASM Update: Qualifying Tools to Measure Functional Status Assessment For HF

Participants selected for the Ambulatory Specialty Model (ASM) for heart failure (HF) will be required to collect and submit data on five MIPS quality measures – including MIPS #337: Functional Status Assessments For HF – starting Jan. 1, 2027.

After reviewing the 2026 Medicare Physician Fee Schedule final rule and measure specifications, the ACC Advocacy team has confirmed that in addition to the Kansas City Cardiomyopathy Questionnaire and Minnesota Living with HF Questionnaire, the following free tools meet reporting requirements for this measure:

  • Veterans RAND Health Survey [VR-12] or [VR-36]
  • Patient-Reported Outcomes Measurement Information System [PROMIS]-10 Global Health or [PROMIS-29]
