The ACC recently launched the PINNACLE Advance Care Planning Toolkit to help providers understand, integrate and document advance care planning into their routine practice. The toolkit helps the cardiovascular care team consistently implement advance care planning in the outpatient setting for all patients. Developed by experts in the field and members of ACC’s Geriatric Cardiology Section Palliative Care Work Group, the toolkit currently offers two resources: the Advanced Care Planning “Terms to Know” and the “Conducting the Conversation” tool.
