Requirements:

  • Registered Nurses – Must have an RN degree and be licensed to practice

  • Clinical Nurse Specialist – Must have an RN degree and a certification in the area of clinical practice and be licensed to practice

To apply, submit your application packet consisting of:

  • Completed Application Form
  • Copy of Your Practicing License
  • Completed and Signed Sponsorship Letter from a Current FACC, AACC or CVT Member
  • Payment of Annual Dues ($55 US) + Application Fee ($25 US) = Total $80 US
