Guidelines Applied in Practice

ACC's Guidelines Applied in Practice (GAP) Program aims to improve the quality of cardiovascular care by bringing ACC/AHA practice guidelines to the point of care. The program consists of a series of projects to develop and test tools and strategies for implementing guidelines. Because each project is focused on a particular guideline, the specifics of each project differ. Key features common to all projects include:
  • Building partnerships
  • Flexibility to allow local adaptation
  • Tools derived directly from the guideline
  • Involvement of caregivers across the continuum of care (i.e., not just cardiologists)
  • Involvement of patients
  • Use of champions/opinion leaders
  • Use of data to change behavior and measure effectiveness of the approach.
