SPECT MPI tests are identified by CPT codes 78451 and 78452.

Patient Look-up Change

Effective May 18, 2013, when entering the health plan member ID for a FOCUS consult, please use only the patient’s new 12 digit Highmark Delaware Unique Member Identifier (UMI) number located on each member’s new card. Prior to May 18th, each patient should receive a new ID card from Highmark Delaware.

The FOCUS system will only accept the existing 6 digit subscriber number until May 17, 2013 after which the existing 6 digit number cannot be used to lookup a patient in FOCUS. Also, the new Highmark 12 digit UMI numbers should not be used in the FOCUS portal prior to the effective date of May 18, 2013.

If a patient has not received a new member cardmust contact Highmark Delaware for assistance by calling the phone number listed on their old card.
