Learning Objectives

Upon completion of this activity, learners should be able to:

  1. Identify knowledge gaps and engage in targeted study to prepare for board certification and ongoing maintenance of certification in Advanced Heart Failure and Transplant Cardiology.
  2. Classify heart failure phenotypes (HFrEF, HFmrEF, HFpEF, HFimpEF, and advanced HF) using clinical presentation, imaging, and biomarker data.
  3. Apply guideline-directed medical therapy (GDMT) for heart failure, including initiation, titration, sequencing, and monitoring of foundational and adjunctive therapies.
  4. Interpret diagnostic testing—including echocardiography and cardiac magnetic resonance imaging, hemodynamic assessment, and laboratory evaluation—to guide heart failure diagnosis and management.
  5. Assess volume status and congestion and select appropriate diuretic and non-diuretic strategies for acute and chronic decompensated heart failure.
  6. Recognize and manage acute heart failure syndromes, including cardiogenic shock and hypertensive emergencies, using evidence-based pharmacologic and mechanical support strategies.
  7. Identify and address comorbid conditions (e.g., obesity, atrial fibrillation, valvular disease, chronic kidney disease, diabetes, pulmonary hypertension) that influence heart failure outcomes.
  8. Determine indications for device-based therapies, including ICDs, CRT, and remote monitoring, based on patient risk profiles and guideline recommendations.
  9. Evaluate patients for advanced heart failure therapies, including durable mechanical circulatory support, heart transplantation, and palliative care integration.
  10. Incorporate multidisciplinary and patient-centered care principles, including transitions of care, self-management education, and shared decision-making.
  11. Apply current ACC/AHA/HFSA guidelines and emerging evidence to clinical decision-making, supporting self-assessment, maintenance of certification, and lifelong learning in heart failure care.
  12. Integrate systems-based practice principles by incorporating quality improvement, patient safety, ethics, cost-effective care, and culturally competent, multidisciplinary collaboration into cardiovascular care delivery.
