CMS Releases Resources For ACCESS Model Implementation
The Centers for Medicare and Medicaid Services' (CMS') new Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) model will begin July 5, 2026. In preparation for the launch, the agency has released a webpage for primary care providers and referring clinicians outlining how the model will work, including the process for enrollment, care updates and co-management of billing.
The ACCESS model aims to test an outcome-aligned payment approach in Traditional Medicare, expanding access to new technological care software and devices that help patients improve their health and prevent and manage chronic disease. Conditions of focus include high blood pressure, diabetes, chronic musculoskeletal pain and depression.
