New HF Accreditation Enhancements Now Live
ACC's Heart Failure (HF) Accreditation program provides a guideline-driven framework to optimize patient care, reduce readmissions, and align processes and quality initiatives with value-based care goals. Recent enhancements make the program even stronger, with new calculated measures, a nearly 40% reduction in Essential Components, and an expansion of the Partners in Care program to include outpatient pharmacies supporting guideline-directed medical therapy access and adherence.
