NCDR-participating facilities that are taking part in the Centers for Medicare and Medicaid Services (CMS) Bundled Payments for Care Improvement (BPCI) Advanced program and have selected quality measure reporting through the ACC for the Alternate Quality Measures Set, must submit the BPCI Advanced Data Release Consent Forum by Tuesday, April 12. The form must be returned to the ACC, not CMS, in order for the data export to take place. Please note that a separate Data Release Consent Forum is required for each registry that is participating in the program.

To access the form, visit the BPCI Advanced page on CVQuality.ACC.org. Under ACC Resources, download the respective Data Release Consent Form. A scanned copy of the completed form must be sent to the ACC for processing. Please contact the ACC at 1-800-257-4737 or via email (ncdr@acc.org) with any questions.
