ACC Vision For Championing Patient Access to Care

The ACC is leading the charge to transform cardiovascular care delivery. Tasked with advocating for cardiovascular patients and providing a professional home for the entire cardiovascular care team, the College is committed to developing and advancing solutions that will optimize the delivery of cardiovascular care, increase access to care and improve patient outcomes and health. However, significant reforms are required within the current Medicare system to achieve the College's vision.

Stagnant and declining payment for Medicare services exacerbates financial uncertainty for health systems and practices and furthers disparities in care delivery. The cost of operating a practice has increased dramatically while physician payment has steeply declined. There is an urgent need to implement policies to stabilize the payment system and recognize the value of clinicians' work.

Appropriate and meaningful patient engagement in care can have a positive impact on health outcomes. The ACC recommends supporting patient care that is facilitated through shared decision-making and merges stratification and evidence-based treatment options with patient preferences.

The College also emphasizes the importance of reducing barriers to early specialty referral and treatment planning to promote systematic patient identification and ensure access to care for patients with suspected or newly diagnosed complex cardiovascular disease.

Ensuring patients have access to high-quality and continuous care is a critical factor in optimizing care delivery and outcomes.

The ACC recommends several actions to make this vision a reality, such as streamlining prior authorization practices; expanding telehealth access through permanent flexibilities; incentivizing effective ambulatory and virtual specialty care; establishing flexibility for aligned incentives across specialties and various health care providers; encouraging accountable, team-based, interdisciplinary multispecialty care; and supporting purposeful operations, data infrastructure and concurrent analytics to manage patient populations in real-time and facilitate timely intervention.

As clinicians make the transition to value-based care, retaining quality will be of utmost importance. To retain quality and support clinicians in providing equitable, value-based care, the ACC suggests achieving quality measurement that drives optimal patient outcomes, providing the care team with actionable data to continually improve and sustain the delivery of quality care.

In addition, the College encourages innovative care delivery and cost-benefit scenarios by embracing appropriate use of advanced diagnostics, new procedures and medications that can improve quality of life, paired with actionable data and tools that empower cardiovascular professionals to address health disparities and social determinants of health.

Decisive action can help keep practices in business and bolster the nation's physician pipeline for the future. The ACC recommends securing financial stability in the Physician Fee Schedule (PFS) Payment System with baseline positive inflationary updates, along with eliminating the $20 million budget neutrality requirement within the PFS and recognizing the negative impact reduced resources has on equitable access to care and patient outcomes.

Act on Medicare Payment Reform

Your ACC is urging members of Congress to co-sponsor the Preserving Seniors' Access to Physicians Act of 2023 (H.R. 6683) and support its inclusion in any upcoming FY24 appropriations package. Importantly, H.R. 6683 would eliminate the impending 3.37% cut to the Medicare Physician Fee Schedule (PFS) that took effect on Jan. 1, 2024.
