Opportunity to Participate in ACP Care Coordination Workgroup

The ACC is seeking one to two members to participate in the American College of Physicians Care Coordination Workgroup. The goal of the workgroup is to develop principles for how patient care is shared between primary care and specialty care practices.

The workgroup intends to publish its findings in a policy paper with the principles for supporting high-value shared care. The workgroup includes a variety of stakeholders including representatives from internal medicine subspecialties, surgery, psychiatry and primary care, as well as patient, family and caregiver representation.

This group meets once a month by phone, in addition to subgroup calls to address topics such as the co-management of a disorder or set of disorders. Workgroup members are encouraged to provide feedback on the conference calls or by editing documents online.
