Please have the following ready as you begin your online application:

  • Sponsorship from two current FACC or AACC members. Select directly through the application or upload a sponsorship letter.
    • Sponsors should be well acquainted with the applicant's professional activities.
    • The letters should be originally written, on the sponsor's business letterhead and highlight the applicant's professional accomplishments and why the applicant warrants the AACC designation.
  • Proof of board certification or successful completion of the ACC’s Certified Cardiovascular Knowledge Examination  for NPs and Pas.
