This month's issue of Cardiology celebrates the successes and advances in the past year that are helping us as a profession to transform cardiovascular practice and improve heart health, while also diving deeper into the treatment of heart failure with preserved ejection fraction (HFpEF), often called the greatest unmet need in cardiovascular medicine.

In the cover story, Eugene Braunwald, MD, MACC, and heart failure specialists Javed Butler, MD, MPH, FACC, and Ileana L. Piña, MD, MPH, FACC, shed light on the research findings that are bringing new hope to patients with HFpEF. They offer their thoughts on the past, present and future of HFpEF and its treatment, including the potential of SGLT2 inhibitors.

Also in this issue, hear from Onyedika J. Ilonze, MD, FACC; Roopa A. Rao, MD, FACC; and Maya E. Guglin, MD, PhD, FACC, on important advances in heart failure, cardiomyopathies, mechanical circulatory support and cardiac transplantation, and catch the major highlights from the new 2021 ACC/AHA/SCAI Coronary Revascularization Guideline. Additionally, read more about the challenges facing clinicians and patients when it comes to treating familial hypercholesterolemia based on a recent survey of ACC members.

Health equity is a long-term strategic priority of the College and we continue to use these pages to provide actionable knowledge that can help members optimize care and outcomes. In this month's Peripheral Matters, Carlos E. Barbery, MD; Alexander C. Fanaroff, MD, MHS; and Howard Julien, MD, MPH, ML, FACC, discuss the significant racial, socioeconomic and geographic disparities in the prevalence and outcomes of peripheral artery disease and explore potential interventions.

Turning to cardiovascular practice, this month's issue looks back on successful ACC Advocacy efforts this past year to ensure patient access to care, stop harmful Medicare cuts and reduce administrative burden. Also get a behind-the-scenes look at what to expect from next month's Cardiovascular Summit in Washington, DC, and learn more about the College's work to help lead the transition to value-based care.

As we close, we want to note the new design of the magazine, part of our ongoing efforts to modernize and improve the reader experience. We'd love to get your feedback on the new look, as well as issues and topics you'd like to see us cover in the coming year. Send your thoughts to cardiologyeditor@acc.org and don't forget to follow @ACCinTouch on Twitter and share your favorite articles using #CardiologyMag.

Happy New Year!
